SV: SetError() #13 - An Exception was thrown into the server



I don't know if this is important, but I do notice that there are double slashes between soap and envelope:
<soapenv:Envelope xmlns:soapenv="http://schemas.xmlsoap.org/soap//envelope/";

This is from SoapUI:
<soapenv:Envelope xmlns:soapenv="http://schemas.xmlsoap.org/soap/envelope/";

Here is the debug file after adding the tags in question.
The result is the same error.

Do we have the correct soap action ?
Is there something else we may be missing in the request or a step required by the API ?
Is anyone familiar with the error and its meaning ?
